Occupational Therapist

We have an immediate opening for an experienced and reliable Occupational Therapist (Contract) in an active 55+ Community located in Kissimmee, Fl. All of the therapist's patients are located in this one community. So travel is minimal.

The therapist will be responsible for:

Visiting the patient's home to perform an evaluation and assessment.

Creating and implementing a treatment plan that will provide optimal therapy for patient.

Visiting the patient's home 2-3 times per week for treatment.

Consistent monitoring of patient with a keen eye for any subtle changes with patient and adjust treatment as necessary.

Keep all important documentation including evaluation, assessment, treatment plan, daily notes and progress notes.

Communication with interdisciplinary team regarding care and treatment.

Be timely in responding and performing Occupational Therapy referrals.

Communicate accurate and detailed information with patient and their families on their rehabilitative care to give them the best outcome for their recovery, well-being, and safety.

Flexible hours, you will largely be in charge of your own schedule working with the patients availability.

Qualifications and skills

• Professional, friendly, encouraging and comfortable working one on one.
• Florida State License as an Occupational Therapist and in good standing with the board.
• Current CPR certification.
• Dependable, reliable, and a team player.
• At least one year therapy experience required.

Physical Therapist

We are looking for a dependable, passionate, and dedicated contract physical therapist. All our patients are located within this one community so there is minimal travel.

We pride ourselves on exceptional patient care.

The ideal candidate is a skilled professional that is passionate about delivering quality care and making a real difference.

Responsibilities

• Assess each patient and develop an appropriate treatment plan with measurable goals and time frames
• Prepare and submit clinical and progress summaries based on identified and attained goals
• Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient
• Guide and consult patient.
• Timely and detailed documentation

Qualifications

• Licensed Physical Therapist with a Master's or Doctor's degree of an accredited physical therapy curriculum
• Certification in Physical Therapy and valid Florida state license to practice
• Currently living in the Greater Orlando area at time of application or plans to live in the area immediately.
• Owns a personal computer/tablet/phone
• Current CPR certification
• At least one year experience in a clinical setting.
• Dedication to our patients
• Excellent Communication skills both written and verbal.
• Comfortable working with patients in their homes.
